Welcome to Clarence & Holakiewicz

Clarence & Holakiewicz (formerly Liz Holakiewicz & Associates) provides Life Care Plans, cost projections, and rehabilitation consulting services for the disabled or catastrophically injured, providing expert testimony when needed. Serving the Southern California area: San Diego, Orange, Los Angeles, Riverside and San Bernardino  Counties. Special requests for life care plans outside of these areas may be considered.


The Life Care Planners at Clarence & Holakiewicz are certified and experienced in developing care plans for individuals who have:

What is a Life Care Plan?

A Life Care Plan is a "dynamic document based upon published standards of practice, comprehensive assessment, data analysis and research, which provides an organized, concise plan for current and future needs with associated costs for individuals who have experienced catastrophic injury or have chronic health care needs." (IARP-IALCP)


The Life Care Plan is a tool that can be used:

  • By a family or individual to financially plan for their disability or health care needs.
  • By a trust administrator or case manager as a guide to appropriate treatment.
  • By an attorney to define the economic damages in a lawsuit.
  • By the insurance industry as a financial planning tool for projecting reserves for high exposure cases.

Adaptive driving and transportation.

What items are included in a Life Care Plan?

The Life Care Plan outlines future care recommendations such as: 


  • Physician care, or office visits with the doctor.
  • Therapy services:  physical therapy, speech therapy, occupational therapy & psychological counseling.
  • Home nursing or facility care and services.
  • Medications.
  • Diagnostic testing:  routine and pre-operative studies.
  • Equipment and supplies.
  • Transportation: drivers training and modified vehicles.
  • Home Modifications.
  • Hospitalizations, surgeries or procedures.
  • Emergency room care or urgent care.
